（2）项目说明：本项目采购设备椎间孔镜经批准拟采购本国产品或不属于国家法律法规政策明确规定限制的进口产品（注：进口产品是指通过中国海关报关验放进入中国境内且产自关境外的产品，含已进入中国境内并在国内市场有销售的进口产品）。

本合同包不接受联合体投标

合同履行期限：合同所约定的全部义务履行完毕之日止

二、申请人的资格要求：

1.投标供应商应具备《政府采购法》第二十二条规定的条件，提供下列材料：

1）具有独立承担民事责任的能力：在中华人民共和国境内注册的法人或其他组织或自然人，投标（响应）时提交有效的营业执照（或事业法人登记证或身份证等相关证明）副本复印件。

2）有依法缴纳税收和社会保障资金的良好记录：提供投标截止日前6个月内任意1个月依法缴纳税收和社会保障资金的相关材料。如依法免税或不需要缴纳社会保障资金的，提供相应证明材料。

3）具有良好的商业信誉和健全的财务会计制度：供应商必须具有良好的商业信誉和健全的财务会计制度（提供2020年度财务状况报告(新成立公司提供成立至今的月或季度财务报表复印件)或基本开户行出具的资信证明）。

4）履行合同所必须的设备和专业技术能力：提供书面声明或证明材料。

5）参加采购活动前3年内，在经营活动中没有重大违法记录：在经营活动中没有重大违法记录：（提供书面声明）。重大违法记录，是指供应商因违法经营受到刑事处罚或者责令停产停业、吊销许可证或者执照、较大数额罚款等行政处罚。（较大数额罚款按照发出行政处罚决定书部门所在省级政府，或实行垂直领导的国务院有关行政主管部门制定的较大数额罚款标准，或罚款决定之前需要举行听证会的金额标准来认定）

3.本项目的特定资格要求：

合同包1(医疗设备)特定资格要求如下:

(1)供应商未被列入“信用中国”网站（www.creditchina.gov.cn）“记录失信被执行人或重大税收违法案件当事人名单或政府采购严重违法失信行为”记录名单；不处于（www.ccgp.gov.cn）“政府采购严重违法失信行为信息记录”中的禁止参加政府采购活动期间((说明：由负责资格性审查人员于投标截止日在“信用中国”网站（www.creditchina.gov.cn）及（www.ccgp.gov.cn）查询结果为准，并将查询结果截图存档。如相关失信记录已失效，供应商需提供相关证明材料））。(2)投标人必须具有《医疗器械生产许可证》或《医疗器械经营许可证》或医疗器械经营备案凭证。(3)若投标产品隶属医疗器械管理的，须提供产品的医疗器械备案凭证；或提供产品的医疗器械注册证、医疗器械产品注册登记表或医疗器械产品生产制造认可表（若投标人已办理二证合一，则只需提供医疗器械注册证）。(4)所投设备椎间孔镜为进口产品且不是制造商直接投标的,投标人须取得制造商或制造商在中国国内代理商/办事处出具的代理证或销售许可证明文件或授权书原件（多层代理关系证明文件应明晰）。(5)单位负责人为同一人或者存在直接控股、管理关系的不同供应商，不得参加同一包号投标或者未划分包号的同一招标项目投标；（投标人出具声明函）(6)为采购项目提供整体设计、规范编制或者项目管理、监理、检测等服务的供应商，不得再参加该采购项目的其他采购活动；（投标人出具声明函）(7)已登记报名并获取本项目招标文件的供应商；（提供购买本项目招标文件的收据凭证复印件）
